This agreement is set between High Voltage Entertainment and the below-named party stating that all terms and agreements have been met prior to the start of the event. A minimum deposit of $150 is required to hold the date of event, and must be received with this contract. You are not booked unless we have this document. The remaining balance is due before the start of the event.
This contract entitles you to 4 hours of music during the times listed above. If the dance does not start at the scheduled time, you will forfeit that time, and money will not be refunded unless it is at the fault of High Voltage. If you would like us to play longer than the scheduled 4 hours, there will be a $25 per half hour fee. High Voltage is not responsible for accidents and the person(s) signing this contract are responsible for any damage done to the equipment of High Voltage.
If the location of event is farther than 60 miles round trip, 50 cents per mile will be added to the total price. If the event is cancelled prior to 60 days of the scheduled event, a full refund will be given. Any “acts of God” causing the cancellation of the contracted events is considered no fault of either party, therefore, making this contract null and void. Further make-up arrangements shall be negotiated.
